Potential Treatment Options for Male Factor Infertility

When a couple is unable to conceive naturally, fertility experts examine both the man and woman for potential causes of the infertility. They then treat the condition. When the cause of the infertility is in the man, it is termed male factor infertility. The causes of Male factor infertility may range from minor abnormalities like slightly reduced counts or motility or complete absence of sperms in the semen. We have a systematic approach to find out the cause and treat it accordingly.

If the cause is minor reduction in count or appearance or motility we may provide simpler treatment options like Intra Uterine Insemination to the couple.

However, if the abnormalities are more severe we need to go for higher treatment options like:

1)   State of the Art procedures like ICSI (Intra Cytoplasmic Sperm Injection) or IMSI (Intracytoplasmic Morphologically selected Sperm Injection) for severely reduced counts or motility (completely absent motility) or morphology.

2)    In cases of absent sperms in the semen we need to find out the cause of absence of sperms (failure of sperm production or obstruction in the sperm transport system) and accordingly provide treatment to the couple.

 In cases of obstruction depending on the level of obstruction, we can provide the couple the options of PESA(Percutaneous Epididymal Sperm Aspiration), TESA(Testicular Sperm Aspiration) or Testicular Biopsy.

Usually the surgically retrieved sperms are few in number and have reduced capacity to fertilize, so again we need advanced procedures like ICSI or IMSI for them.

ICSI – It is an advanced ART procedure where from a grossly abnormal semen sample we select the relatively normal forms under higher magnification (400 times) and inject it into the oocyte to ensure higher fertilization.

IMSI – It is a further advanced procedure where the sperms are selected under still higher magnification (7200 times) to ensure that we select sperms as normal as possible for injections for higher success rates.
