  What is PCOS?  

PCOS or Polycystic Ovarian Syndrome is a hormonal condition affecting the ovaries. In a menstrual cycle, there are usually five follicles on an average that start growing and one of these follicles will mature to release egg at ovulation. However, in a woman affected by PCOS, there is an imbalance in the hormonal system which makes multiple follicles to grow  but none release an egg thereby causing infertility.

  Causes of PCOS  

PCOS can be caused by a combination of multiple factors. Research has shown that genetics plays a role, as this condition is known to run in families. Secondly, when the woman have high levels of insulin in blood due to peripheral insulin resistance or obesity, it can cause PCOS. The increased insulin creates a hormonal imbalance, which produces more of the LH (luteinsing hormone) compared to FSH (follicle stimulating hormone). This results in body producing more testosterone (male hormone) compared to estrogen (female hormone). The increased testosterone level prevents process of ovulation.

  How is PCOS Diagnosed?  

PCOS is usually difficult to diagnose, as symptoms vary largely between patients, and the symptoms can come and go in a patient. The doctor will have to exclude all other causes that can cause such symptoms.  Diagnosis of PCOS is made by presence of the following criteria:

  • Symptoms like irregular menses or no menses
  • Signs of increased male hormones like acne, excessive male pattern hair growth. Signs of insulin resistance like hyper pigmentation of skin, obesity.
  • Doing an ultrasound scan for checking for the polycystic ovaries
  • Laboratory tests like testosterone levels, DHEA-S
 

Treatments for Getting Pregnant with PCOS

 

PCOS is considered incurable, but certain treatment options can improve chances of becoming pregnant. Here are some of the important treatment options.

 

1) LOD (Laparoscopic Ovarian Drilling) is a surgical procedure on the ovaries that is usually recommended when the woman is not responding to medication options. LOD will destroy the ovarian tissue that is producing testosterone. The effect will not last, but it can be there long enough to restore hormonal balance until the woman conceives.

2) Clomiphene, a fertility medication is usually the first treatment option, as it stimulates ovulation. When comiphene does not work, another drug, gonadotrophins is often prescribed. However, this medication can cause overstimulation of ovaries that can result in multiple pregnancies.

3) A woman who is clomiphene resistant or obese is usually given metformin, an anti- diabetic medication. This drug increases the sensitivity of the body to insulin, so that the level of insulin and testosterone in the body decreases which helps in spontaneous ovulation.
